Dr. Prakash Patel is a respected physician, healthcare entrepreneur, and community leader with over 20 years of experience in pulmonary medicine, critical care, sleep medicine, and healthcare operations.
As the Founder and President of Jeevan Jyoti Adult Day Center, Dr. Patel is dedicated to enhancing the quality of life for seniors and supporting families through compassionate, community-centered care. His vision for Jeevan Jyoti is rooted in the belief that every senior deserves dignity, companionship, meaningful engagement, and access to high-quality support services within a safe, welcoming, and respectful environment.
Dr. Patel is also the Founder of Smyrna Pulmonary & Sleep Associates (SPSA), a multi-location pulmonary and sleep medicine practice serving communities across Middle Tennessee. Through his leadership, SPSA has grown into a comprehensive healthcare organization focused on clinical excellence, innovation, and patient-centered care.
Through Jeevan Jyoti Adult Day Center, Dr. Patel combines his extensive healthcare expertise with a longstanding commitment to community service and culturally sensitive care. His mission is to create a supportive environment where seniors can remain active, socially connected, and empowered while helping families navigate the challenges of caregiving with confidence and peace of mind.
Dharmesh (Dan) K Patel,MD, Chief Advisor & Member, Jeeevan Jyoti Adult Day Center
Dr. Dharmesh K. Patel is a physician, business owner, investor, and operator with extensive experience spanning healthcare, hospitality, real estate, and retail industries.
Dr. Patel earned his Doctor of Medicine degree from the University of South Gujarat in 1999 and completed his M.D. training in the United States in 2003. He has been practicing Hospital Medicine in Chicago, Illinois since 2004, bringing over two decades of clinical experience, healthcare leadership, and patient-centered care to his professional work.
In addition to his medical career, Dr. Patel has successfully developed a diverse business and investment portfolio that includes hotel properties, residential and commercial real estate, retail ventures, and healthcare-related enterprises. His background in business operations, strategic growth, and organizational leadership provides valuable expertise in building sustainable, community-focused healthcare services.
Dr. Patel is also a Founding Member of Love N Care Adult Day Care Center in Downers Grove, Illinois, where he has been actively involved in the administrative and operational aspects of adult day care services since 2023. His hands-on experience in adult day care operations brings important insight and leadership to the vision and growth of Jeevan Jyoti Adult Day Center.
As a Co-Founder of Jeevan Jyoti Adult Day Center, Dr. Patel is committed to helping create a compassionate, high-quality, and community-centered environment where seniors receive meaningful support, companionship, and dignified care while families gain trusted resources and peace of mind.
Outside of his professional responsibilities, Dr. Patel enjoys playing volleyball, spending time with family and friends, and volunteering with nonprofit organizations.

Jignesh Patel is a Computer Engineer and healthcare technology professional with more than 14 years of experience in the healthcare industry, specializing in technology-driven solutions that improve operational efficiency, patient care, and healthcare service delivery.
Throughout his career, Jignesh has worked closely with healthcare organizations and multidisciplinary teams, gaining a deep understanding of the critical role that compassionate, reliable, and community-centered care plays in improving health outcomes and patient experiences.
Beyond his professional career, Jignesh is deeply committed to community service and volunteer leadership. He has been an active member and volunteer with GAMT (Gujarat Association of Middle Tennessee) for over six years, contributing to numerous cultural, charitable, and community-focused initiatives. He also serves with ICSSS (Indian Community Senior Support Service), supporting programs dedicated to improving the well-being, engagement, and quality of life of senior citizens.
As a Managing Member and Co-Founder of Jeevan Jyoti Adult Day Center, Jignesh is helping lead a new chapter in senior care services. His vision is to create a warm, safe, respectful, and welcoming environment where seniors receive compassionate care, meaningful social engagement, and a true sense of belonging.

Manish Patel has been a proud resident of Nashville, Tennessee for over 25 years and has remained deeply connected to the Middle Tennessee community through leadership, business, and community service.
Manish earned his Bachelor of Engineering degree in Computer Science and Engineering from Bangalore University, India. Following a successful corporate career, he expanded his professional journey into entrepreneurship, business ventures, and real estate investments.
Driven by his passion for real estate, community growth, and relationship building, Manish obtained his real estate license and currently serves clients across Middle Tennessee through Compass Real Estate, helping individuals and families navigate residential and commercial real estate opportunities.
Since moving to Nashville, Manish has actively contributed to numerous community organizations and is widely recognized for his approachable leadership style, strong community relationships, and dedication to service. He currently serves as the President of GAMT (Gujarati Association of Middle Tennessee), one of the oldest and most respected cultural organizations in the region.
Coming from a medical family background, Manish has a strong appreciation for the importance of compassionate elder care and family-centered support. Jack Patel brings decades of leadership, operational expertise, and community service experience to Jeevan Jyoti Adult Day Center.
With more than 30 years of experience as a System Supervisor at Nashville Electric Service, Jack Patel has built a strong foundation in operations, leadership, customer service, problem solving, and organizational management. In addition to his professional career, he is a successful business owner and entrepreneur with extensive experience in business operations, property management, and community development.
Jack has been deeply committed to serving the community throughout his career. As President of Shree Umiya Dham, he has had the privilege of supporting and strengthening the local community through leadership focused on professionalism, service, compassion, and sustainable growth.
As Co-Founder of Jeevan Jyoti Adult Day Center, Jack’s vision is to create a compassionate, culturally welcoming, and community-centered environment where seniors can thrive. He is passionate about building a place where elderly individuals can enjoy companionship, meaningful daily activities, entertainment, wellness programs, and cultural celebrations in a safe, respectful, and engaging setting.
